I understand that all sales are final and are not eligible for transfer or exchange or sharing, and can only be used by the registered name holder(s). I voluntarily agree to participate in the activities being provided by MADE TO MOVE STUDIO (CT0070543-P).
I confirm that I have obtained a medical practitioner's express approval and clearance to participate in the session. I confirm that at the time of participating in the session I am not unwell, not suffering from any form of mental and/or physical injury; and/or am not pregnant or post natal without a doctor's clearance to participate in Pilates training. Note: Should you not be able to give all of the confirmations above, you must not participate in the session.
I understand that if for any reason I am unable to or think it unwise to engage in any activity being suggested by Made to Move Studio, I am under no obligation to engage in such activity nor will I hold Made to Move Studio, or its respective agents (i.e., directors, officers, employees, independent contractors, service providers and any third parties acting on its behalf), liable for any damage, injury or harm sustained from choosing to engage in any activity.
I also understand that there are unanticipated risks during any activity. I hereby assume all risks of injury to me and my property, which may be sustained in connection with these activities. I agree that, in the event Made to Move Studio determines I need medical or psychological attention, Made to Move Studio has the authority and discretion to contact emergency services. Any costs incurred for such services are my sole responsibility.
I understand that my participation in any activity is at the discretion of Made to Move Studio. If, in the opinion of Made to Move Studio, I am unable to continue in an activity for any reason, I may be asked to stop or leave.
I, on behalf of myself, my personal representatives, heirs, executors, administrators, agents and assigns hereby grant Made to Move Studio the perpetual, non-exclusive, royalty and compensation free, right and license to record my participation and appearance in classes, workshops, or events on digital or film photography, video, audio or any other medium and use my name, likeness, and voice to reproduce, distribute, publicly display, in print, electronic or any other medium, the same for promotional purposes, advertising, video, web, new media, or other formats for which Made to Move Studio wishes to utilise.
I hereby acknowledge that I have read, agree, and understood that I am giving up substantial rights, including my right to sue. I have had the opportunity to ask Made to Move Studio any further questions I may have had. I also represent that I am of legal age and freely and voluntarily and unconditional release of all liability to the greatest extent allowed by law from MADE TO MOVE STUDIO and all its subsidiaries, staff, instructors and subcontractors.
The term of this agreement is indefinite.
